Language Waiver Application Form

To be considered for a language waiver on disability grounds, please complete the application form below and upload the supporting documentation relevant to your application.

Supporting documents required to accompany your application:

1. Copy of your Department of Education Certificate of Exemption, signed and stamped by your School Principal
2. MIC Language Waiver School Declaration form, completed and signed by your School Principal
   and a copy of Educational Psychologist Report (only if relevant to your application)
