CAO Concession Sports Scholarship Application

Please read the information below carefully before completing and submitting the application form.
- All fields must be completed in one session (i.e. this form cannot be saved and revisited at a later time). Incomplete applications will not be considered. Characters aside from letters and numbers in any field may cause an error i.e. punctuation marks.
- If prompted, complete the reCAPTCHA task set and/or tick the reCAPTCHA box [this is a security measure]..
- Forms must be submitted before the application deadline of 1 May 2024. Late applications will not be considered.
- Once submitted, changes cannot be made to your application; you are advised to keep a record of the information you add.
- Canvassing will automatically disqualify applicants.
- If your application is submitted successfully, you will receive an email confirmation. Please keep this for your own records.
