Important Information

Please refer to MIC Sports Schemes Terms & Conditions before applying.

Please read the information below carefully before completing and submitting the application form.

  • All fields must be completed in one session (i.e. this form cannot be saved and revisited at a later time). Incomplete applications will not be considered.
  • Forms must be submitted before the application deadline of 1 May 2020. Late applications will not be considered.
  • Once submitted, changes cannot be made to your application; you are advised to keep a record of the information you add.
  • Canvassing will automatically disqualify applicants.
  • If your application is submitted successfully, you will receive an email confirmation. Please keep this for your own records.
